چکیده

Patient 1 A 16-year-old girl was seen for a second opinion on renal FMD. Hypertension was diagnosed at the age of 13 years and initially treated medically; however, bilateral renal artery angioplasty was subsequently performed for poorly controlled blood pressure. She subsequently underwent placement of a drug eluting stent in the right renal artery for restenosis. Although she initially improved, there was gradual worsening of her blood pressure control. Noninvasive testing was consistent with severe restenosis of the right renal artery stent. Renal arteriography revealed a severe stenosis estimated at 80% in the right renal artery (Figure 1). There was severe narrowing noted at the ostium, resulting in a 70 mm Hg gradient across the lesion. Intravascular ultrasound confirmed stent fracture as the cause of narrowing (Figure 2). The patient was referred for aorto-renal bypass grafting with venous conduit and did well postoperatively.

Background: Renal artery stenosis, which is defined as narrowing of one or both renal arteries or their branches, is one of the most common causes of secondary hypertension. The majority of renal vascular lesions show either fibromuscular dysplasia or atherosclerosis. Fibromuscular dysplasia is a common cause of renovascular hypertension in younger patients, and 90% of patients are young wome...

Recognizing and managing fibromuscular dysplasia.

Fibromuscular dysplasia typically occurs in young women and most commonly presents with hypertension, transient ischemic attack, stroke, or an asymptomatic cervical bruit. The disease is nonatherosclerotic and noninflammatory and most often affects the renal and carotid arteries, although almost any artery can be involved. On angiography, affected blood vessels characteristically resemble a str...

Renal artery rupture following cutting balloon angioplasty for fibromuscular dysplasia: a case report

INTRODUCTION Angioplasty with the use of cutting balloons has been suggested by some case reports and small series for the treatment of renal artery stenoses that are resistant to conventional balloon catheters. Based on this limited experience, the use of this technology has been suggested as safe. Herein, we report a renal artery rupture following angioplasty with a cutting balloon. The compl...

Surgical revascularization of bilateral renal artery stenosis due to fibromuscular dysplasia

Fibromuscular dysplasia (FMD) is a noninflammatory disease affecting small- and medium-sized arteries of the renal and the carotids. It affects the renal arteries in nearly 60%-75% cases. The primary clinical manifestation of renal FMD is hypertension. Medial fibroplasia represents the most common dysplastic lesion. We report two cases who presented with hypertension and renal insufficiency and...

Intimal Fibromuscular Dysplasia of Renal Artery: A Rare Case of Hypertension

Hypertension has a long list of primary as well as secondary causes. Fibromuscular dysplasia is amongst the rare secon dary causes of hypertension. Intimal fibroplasia as a pathologic cause occurs in less than 10% of patients with fibromuscular dysplasia. We report a rare case of hypertension due to intimal fibromuscular dysplasia of renal artery, treated successfully with renal angioplasty an...
